Indiana Direct Primary Care is committed to keeping the Fort Wayne community healthy. We start with nutrition and exercise as the foundation of wellness. We partner with our members to offer both integrative and traditional medication options.

In-House Pharmacy

Affordable & Convenient

Everyone's time is valuable. We save our members time by filling many medications and supplements in the office during your visit. Not only is it convenient, but we also offer them to you at wholesale cost saving you money as well.

Fitness Center

Fitness & Nutrition

While many doctors' offices can recommend diet and exercise we take it to the next level. Attached to our office is our state of the art, 24/7 fitness facility that is available to members only.

Membership Details

We offer comprehensive care for a reasonable monthly fee. We DO NOT accept insurance payments. Certain HSA and FSA plans can be used for membership fees. Please consult your plan for details. For external services such as labs and radiology we are happy to provide them with your insurance information if you have it.

*Initiation Fee: $99

Adults: $99/month
Children 16+: $79/month with adult parent membership
Children 6-15: $25 per visit with adult parent membership
Children 0-5: $30 per visit with adult parent membership

*Fitness facility available to members 16 years of age or older

* We are happy to see kids for all things including well visits but unfortunately we are unable to offer routine childhood vaccinations at this time. (age 0-5)

Cancellation Policy:

30 days advance notice is all that is required to cancel your membership
Re-initiation fee: $250 (we hate to see our members go and will do everything we can to help you stay)

Missed/Cancelled Appointments:

Do to the nature of longer appt times we reserve the right to charge a $25 fee for missed appointments for all services
We reserve the right to charge a $25 fee for any appt with cancellation less than 24 hours notice (we appreciate urgent things can happen and will waive this fee for extenuating circumstances)

Appointments

Our members enjoy a "one-stop shop" experience taking care of their medical appointments, medications, labs, nutrition, and fitness. Appointments are available for:

Office visits: Extended office visit times allow our members to not feel rushed and spend more time with the doctor and less time in the waiting room.
Routine Well Woman and Well Male exams (including hormone replacement therapy and testosterone replacement)
Sports physicals
Well Child visits
Virtual visits: Select visit types can be completed from your home or office or from your cell phone on the go.

In-Office Procedures

We perform many procedures at no additional cost.

These include but are not be limited to:

EKG
Skin excisions and biopsies*
Toenail removal
Joint injections
Sutures and suture removal
Basic wound care
Pap smears*

In-Office Testing

We perform various rapid testing for the convenience of our members. These include but may not be limited to:

Strep screening*
Mono*
A1c*
Blood sugar
Urine dip
Influenza*
Pregnancy
Rapid Covid Antigen*

Labs

We partner with a local laboratory to offer inexpensive blood work for those without insurance. We offer all labs at wholesale cost including a full wellness panel* for just $50.

If you have insurance, we can draw the sample in our office and the laboratory can bill your insurance directly.

*Wellness panel: Complete blood count, electrolytes, kidney/liver/thyroid function, diabetes screening, cholesterol profile

In-Office Pharmacy

We offer many medications and supplements for high blood pressure, cholesterol, diabetes, and more at wholesale prices. Many antibiotics and cold medications are stocked for a one-stop shop experience to help you save time and avoid the drive and lines at the pharmacy when you already don't feel well.

Naturopathic Options

Want to try non-traditional options? We can help with that too. We offer essential oils and other supplements for many different health conditions that help improve our members overall health.
